Community Health Needs Assessment

The Story County Quality of Life Alliance (SCQLA) conducts a health and human needs assessment every five years as requested by the Iowa Department of Public Health. This assessment guides local educational programming and services.

 Opens in new windowThe 2025 Community Health Needs Assessment incorporated data from multiple sources, including primary research (through the PRC Community Health Survey and PRC Online Key Informant Survey), as secondary research (vital statistics and other existing health-related data). It also allows for trending and comparison to benchmark data at the state and national levels.

Prioritization of the needs identified in this assessment was determined based by overlaying data uncovered through the population survey and secondary data review with the input received from providers and other community leaders (representing a cross-section of community-based agencies and organizations) as part of the Online Key Informant Survey.

United Way of Story County uses the information from this report to guide where we focus our efforts and investment. The data helps us see where the biggest needs are and where we can make the most impact—whether that’s improving access to mental health care, addressing food insecurity, or supporting families facing housing challenges. By using this information, we can work alongside community partners to create lasting solutions and build a stronger, healthier Story County for everyone.
